Electronic FMCSA Submission

Your DOT medical card is submitted electronically to the FMCSA, which then forwards it to the DMV. No paperwork is required on your end.

What to Expect at Your Visit

  • Before your appointment, we will send you a health history questionnaire via e-mail or text. To save time, please complete it before your visit.
  • Please wear comfortable clothing for your DOT physical appointment. Avoid wearing clothes that are too tight or restrictive. Loose-fitting clothing is best for the examination.
  • The Medical Examiner’s Exam Includes The Following:
    1. Looking over your medical history
    2. A urine test will be used to evaluate you for any potential medical issues. This is not a drug test.
    3. A relatively basic physical exam that will consist of checking your measurements such as heart rate, breathing, height/weight, eyes and ears, etc.
  • Our Medical Examiner will fill out your Medical Examination Report for Commercial Driver Fitness Determination form and will issue Medical Examiner’s Certificate/DOT Medical Card if all is in order. Both of these documents will be completed at your exam and will be provided to you before you leave.
  • If you would like, or your company requires it, we can fax these documents over to your company on your behalf.
