Do DOT Physicals Require a Drug Test?

A very common question we get asked is whether or not your DOT Physical Exam will also include a drug test. It does not. When it comes to drug testing for your CDL, that would be handled by your employer typically during the pre-hire process, or during your employment.

We do not perform drug testing or reporting during any of our DOT physical exams and it is not a requirement to receive your medical clearance.

If You Don’t Do a Drug Test, Why Collect a Urine Sample?

We use a simple urinalysis to screen for underlying medical conditions that may affect your ability to safely operate a commercial vehicle, and nothing more.

What Exactly Are You Screening For?
  • Glucose Levels: To screen for diabetes.
  • Protein: To detect kidney disease or other conditions affecting the kidneys.
  • Blood: To identify urinary tract infections or kidney issues.
  • Specific Gravity: To assess the concentration of urine, which can indicate dehydration or other conditions.

Can I Fail The Test?

The urine analysis we do is not a simple pass/fail as a drug test may be. If something pops up that may be concerning, it may require further evaluation before clearing you medically to operate a commercial vehicle.

However – if you have a primary care physician and that you regularly see, chances are you will already be aware of any underlying medical conditions.

It is worth noting that if you have diabetes, you should also see our page on getting a DOT Physical with diabetes.
